CASE MANAGEMENT
The Case Management Program for the Frail Elderly coordinates community services to help seniors remain in their own homes. The Case Manager and the Interdisciplinary Team (local in-home care professionals) recommend appropriate services to the client and family to help support that individual in their own home.

WHO? Eligible seniors must be 60 years of age and reside in Cass, Fremont, Harrison, Mills, Montgomery, Page, Pottawattamie, or Shelby counties in southwest Iowa and need assistance with at least two activities of daily living. Activities of daily living include dressing, grooming, bathing, toileting, transferring, and ambulation.

WHAT? Case Management is the coordination of in-home services by a caring professional who assesses the senior and with the help of the interdisciplinary team recommends services to the senior and their family. This is a client-driven service. Clients determine which services they do or do not want to receive.

- Adult Day Care
- Assistive Devices
- Chore Services
- Personal Emergency Response Systems
- Home Delivered Meals
- Home Health Aide Services
- Homemaker Services
- Home/Vehicle Modification
- Mental Health Outreach
- Nursing Care
- Nutritional Counseling
- Respite
- Senior Companions
- Transportation
- Consumer Directed Attendant Care

HOW does our program work? You and the case management team will identify the services which can assist you in remaining in your home. You and your family decide which services you will accept. One person from the case management team will be your contact. All information remains confidential. Volunteers will be available to assist you with independence issues.
 
WHAT are the advantages for you and your family? Informed choices about services in the community. Assistance in remaining independent for as long as possible. One contact person for all your concerns and questions. Coordination of available help without duplication or gaps. Family and in-home services will be encouraged.
 
WHAT does it cost? The case management program is available at no charge. (This includes a comprehensive assessment and service plan.) Volunteer services will be available at no charge. Referral agengies or organizations will charge for their services. Eligible clients will receive services paid for by Elderly Waiver, Senior Living Program or  Family Caregiver Support Program.
 
 
 
PAYMENT SOURCES AVAILABLE
 
 - Private pay by client and/or family.
 
-  Family Caregiver Support (Older Americans Act) Client must be 70 or older, have a primary caregiver and need assistance with two Activities of Daily Living. Services available for payment include legal assistance, respite, assistive devices, medication management and caregiver counseling.
 
-  Senior Living Program (State of Iowa) Client must be 60, need help with at least 2 Activities of Daily Living and income below $25,770 yr. Senior Living Program clients will be eligible for $300-$400 per month in services. See potential services listed above. The Senior Living Program is designed to assist seniors in maintaining their independence and quality of life in their own homes.
 
- Home and Community Based Medicaid Elderly Waiver (Iowa Dept. of Human Services) Clients must be 65 years of age, meet nursing facility level of care, and have income below $1590 per month and resources of $2000 or less. For couples, the spousal impoverishment provisions apply. Potential clients must complete a Medicaid application. Eligible clients may receive services up to $1052.00 a month and a Medicaid card for physician and prescription services.
